PRIVACY NOTICE
( FOR EMPLOYEE )
Eastern Polymer Group Public Company Limited, hereinafter referred to as the “Company”, respects the privacy rights of its employees; the Company is aware of the importance of personal data and the protection of personal data that must be adequately provided. To prevent infringement of the privacy rights of the personal data subject, which is our responsibility, therefore; the Company hereby prescribes and notifies the privacy for processing personal data to the personal data owner for their acknowledgement regarding the details related to the collection, use or disclosure of personal data as follows:
1. The purpose for collecting, using, or disclosing personal information
The Company will collect, use, or disclose personal information as necessary for the Company’s human resource management for the following purposes:
1.1 To be used in the preparation of Employment Contracts, Employment Conditions Agreement, Compliance with the Employment Contract, Employee Registration, Employee Orientation, Remuneration Payment, Job Accident Insurance, Compliance with Regulations, Codes of Conduct, Job Assignment, Employee Job Transfer and Promotion, Job Relocation for employees to work in a group of companies or its affiliates, Training, Performance Appraisal, Administration and Supervision of Employee Health and Safety, Labor Relations Disciplinary, and for being used in compliance with the Company’s human resource management regulations.
1.2 To comply with laws, regulations, and orders of those with legal authority, such as the law on Labor Protection Act, Labor Relations, Social Security, Safety, Occupational Health, and Environment at Work and Control of Occupational Diseases and Environmental Diseases and other related regulations.
1.3 For the legitimate benefits of human resource management, data analysis and manpower allocation, personnel development, promotion, transfer, bonus payment, wage increment, retirement, medical welfare provision, medical records, insurance welfare arrangement and other welfare of employees such as hospitals, cafeteria, gym, recreational activities, carrying out activities in various matters for employees, internal correspondence, contact with outsiders, various registration operations, authorization, preparation of certificates, publication of documents, report preparation, submitting information to government and/or regulatory agencies, verification of identity and validation of information received from employee, analyzing and building a database of work history and/or suggestions, communication, news release and public relations, improving the working environment, provision of facilities, data security, creating an account for user, identification for accessing work systems and accessing information systems security, preventing accidents and crimes, investigating and handling complaints and corruption, and lawsuits or disputes.
1.4 To prevent and suppress physical harm to the life or health of employee, such as emergency contacts, communicable disease control and prevention.
1.5 For the performance of duties in the mission for the benefit of the public or to perform the duties of the granted powers of the state.
1.6 For communication and public relations activities within the group of companies such as training, seminars, recreational activities, activities to help society, meetings, participating in booth activity, preparation of various annual reports, etc.
2. Personal Data Processing Basis
The Company will process personal data based on lawful data processing basis as follows:

3. Personal information that is collected, used or disclosed includes:
3.1 Information and documents related to the recruitment and hiring process such as resume, Curriculum Vitae (CV), Cover Letter, Company Job Application including, supporting documents and interview assessment comments.
3.2 Contact information of employee such as name, surname, domicile address, current address, current residence information, phone number, email, social media information and contact person(s) who are non- relative and contact person(s) for emergencies.
3.3 Employee information such as date of birth, age, height, weight, blood type, nationality, race, religion, gender, marital status, military status, ordination history, health information, interests, opinions, and other details such as referral information, information of relatives or friends who work in the Company, and readiness in travelling to other provinces, etc.
3.4 Information about family members or dependents of employee who are eligible for welfare benefits in accordance with the Company’s regulations and human resource management regulations, such as information about spouses, information about the child, information about parents, and beneficiary person. However; prior to providing the information, the employee must notify this privacy notice to such person for their acknowledgement.
3.5 Photographs or motion pictures for identification and safety in the workplace, including the Company’s activities.
3.6 Information about education history, activities during education, capability, potential development and other employee qualifications such as education level, academic year, education qualification, GPA, institution/university, training history, test results, professional qualifications, the ability to use foreign languages and computers, the ability to drive a vehicle with a license document; and other talents based on references provided by the employee.
3.7 Information about the employee’s past working experience includes company name/address, working hours, position, details of salary and remuneration, welfare received, nature of work and the reason for leaving.
3.8 Information about employee characteristics such as personality, behavior, attitude, aptitude, skill, leadership, the ability to work with others, emotional intelligence, and corporate commitment that may be obtained through observation and analysis during the job performance or participation in activities.
3.9 Information chosen by the employee to share and disclose information through the Company’s systems, applications, tools, questionnaires, assessments, and documents.
3.10 Copies of documents that can be used to identify the employee, such as ID cards. Passport, driver’s license, house registration, and other documents issued by government agencies.
3.11 Information which is necessary for the performance of the employment contract, undertaking welfare benefits, analysis and administration including employee support after retirement and compliance with the law.
3.12 Information about criminal records for considering of the suitability of work and to maintain the legitimate benefits of the Company or a third party, which will be conducted via with the method as prescribed by law.
3.13 Information about accidents, whether the employee has an accident during work or due to work and other accidents.
3.14 Information required for reporting to a regulatory body such as the Ministry of Labor, the Stock Exchange of Thailand, and the Securities and Exchange Commission (SEC).
3.15 Employee financial information includes wages, salaries, income, taxes, provident funds, bank accounts, borrowings, and tax exemptions or deductions.
3.16 Employee information related to social security, labour protection, labour relations, rights and benefits, welfares, and benefits that employee receive or are entitled to receive in accordance with the Company’s regulations and human resource management regulations.
3.17 Record of attendance and time of work, overtime, and employee leave.
3.18 Employee’s work history, job titles, meeting attendance, and comments.
3.19 Information relating to complaints, grievances, investigations, and disciplinary punishment.
3.20 Health information and/or health examination results such as weight, height, congenital disease, colour blindness, food allergy information, drug allergy information, blood type, medical certificate, medical history, dispensing history, receipts of medical expenses for labour protection and welfare related to medical treatment of employee, employee competency assessment including complying with relevant laws to study and analyse employee health data for other appropriate management purposes.
3.21 Biological data such as fingerprint and face image data are to be used to identify and verify the identity of the employee’ crime prevention and maintain the legitimate benefits of the Company.
3.22 Information on usage and access to information systems, computers, operating systems, applications, network systems, electronic devices, e-mails, and websites, including the use of cookies to store information in order to improve and develop the website and to comply with the Company’s information technology policy and related laws.
3.23 Information which was collected from company activities such as participation in events, survey responses, and assessment responses.
3.24 Other personal data for lawful purposes or information disclosed to public with the applicants’ express consent to achieve the objectives related to job recruitment.
4. Storage Period
4.1 The Company will keep personal data for the period necessary to achieve the specified objectives that considering the necessity of each type of data practice and after the expiration of such period. The Company will further destroy or delete personal data by appropriate means as determined by the Company.
4.2 The Company shall retain employee’s personal information throughout the period of employment and ten years after the termination of employment to achieve the employee privacy objectives.
4.3 The Company will provide an audit system for deletion or destruction of data after the expiration of the retention period, or unrelated to or beyond the necessity for collecting that data.
5. Disclosure of Personal Information to Third Parties
5.1 The Company may disclose the employee’s personal information to its affiliates and third parties, such as transfer considerations, background checks, testing of qualification, and ability to collect and process other personal data to achieve the objectives of this Privacy Notice for Employee.
5.2 The Company will require the recipient to take appropriate employee data protection measures, personal process data as necessary, and take steps to prevent the disclosure of information or unlawfully disclosing information.
6. Employee Rights
Employee will still have the right to their personal information that the Company collected, including:
6.1 The right to be informed of the details before collecting the employee’s personal information.
6.2 The right to request a copy or transfer their personal data to a third-party data controller.
6.3 The right to object to data collection without consent.
6.4 The right to request for suspension of the use of information.
6.5 Right to request for the removal and destruction of the information that no longer required to be stored.
6.6 The right to request to keep employee’s personal information up to date.
6.7 The right to withdraw consent for information collected based on consent.
6.8 The right to complain if the Company uses employee’s personal information unlawfully.
However, the refusal to provide information or withdraw consent is required for the aforementioned necessity and purpose or to perform an employee’s request. This may prevent the HR management service from operating entirely. This withdrawal of consent will not affect the collection, use or disclosure of information for which consent has been given.
7. Measurement to Maintain the Security of Personal Data
To maintain security and prevent the loss, access, use, alteration, correction, or disclosure of personal data without authority or wrongdoing. The Company has provided a system for collecting personal information. There is an access control mechanism, security measurement is in place, and the Company will arrange for regular reviews of measurement to effectively maintain physical, administrative and technical security.
